Access to Medical Records

If you wish to access your medical records please see the following leaflet for more information about how to make an application and the costs involved.

All applications need to be discussed and approved by your GP, so please provide as much information as you can about the aspect of your records you wish to access and why.

Please use the following form for all access requests.

Please note that there is no facility for immediate access to medical records.
If you wish to access the medical records of a deceased person you may be able to do so if you are;

  • A personal representative
  • An executor
  • Or someone who has a claim resulting from the death (this could be a relative or another person)

However, it is very likely that the Medical Record will no longer be held at the surgery.
